Fall 2009 Registration

UPDATE: Club registration is now passed.  Please talk to your coach or team manager about registering.  

You can register your player with the state ONLINE using the the Utah Youth Soccer website.  BRING the printed registration form to club registration to save time.

What do you need to register?

Player Code of Conduct
Parent Code of Conduct
Commitment Card

Bring a copy of:

• a copy of current health insurance card
• Birth certificate (hospital, baptismal or religious certificates will not be accepted)
  -If you have registered for a team in Distrct 7 during a previous season, check the following web site to see if District 7 already has your birth certificate. If they have you listed, you don't need to bring a new one to registration. Birth Certificate Query

Also bring:

• 1”x1” Picture (head shot)
• An Idea of what size of uniform the player needs (we will have jerseys for the players to try on at registration)
• A check book to write a check for Infinity Soccer costs - please put players name in the MEMO section (you can also pay by credit card)
